Job Title: Medical Social Worker Profession: Social Services Specialty: MSW (Masters of Social Work) Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: 1 year of acute care experience required License: California state license required Certifications: BLS, LCSW, MSW Must-Have: - 1 full year of acute care hospital experience - Previous charge experience preferred - Experience with charting systems, particularly Epic - Ability to work with diverse patient populations Description: The Medical Social Worker will be responsible for managing a caseload of 20-23 patients. The position requires conducting initial assessments within 24 hours. Coordination of discharge plans and post-acute care needs is a key responsibility. The social worker will work closely with various medical teams and insurance case managers. The position involves developing care plans to meet patient and caregiver needs. Collaborating with internal and external team members is essential for quality care coordination. Social Workers in this role will not have specialized experience in behavioral health or emergency departments. The workweek consists of variable days off and operates seven days per week. Efforts will be made to schedule consecutive days off when possible. Travelers may be required to float between facilities as necessary.
